They only spawn with bosses, so you'll need to locate one to encounter a loyal henchman following them around. Every boss has two henchmen following them. There are a few different bosses you can find wandering around the island, including Human Bill, Beach Brutus, and Hush. All bosses rotate between various locations in each round, so there's no way to know where each one will appear. The only indication you have to go off is a skull with a crown icon that appears on your map once you're fairly close to a boss. This icon always marks a boss's location, and thus some henchmen too. The special mini bosses, Yuki and Gogo, also spawn with some henchmen. To find them, you have to locate a Forsaken Vow Rift Gate. You earn a Forsaken Vow Blade for entering one, and can use it to take down both Kill Bill bosses and two henchmen after you exit. Players who possess a boss Medallion can activate it occasionally to summon two henchmen through a special rift. This ability has a long cooldown and is entirely up to the enemy players' discretion to use, though, so you'll have to wait and hope that it actually happens. This method of finding a henchman is a lot less reliable because of this, so it's best to focus on finding an NPC boss to get it done.
